1. Who Controlls This Policy

• Booking & account data: appointment preferences, service selections, account credentials, notes you submit, and communication preferences.

• Payment data: payment card details and billing information (typically processed by payment processors; we do not store full card numbers on our public Site when processors handle tokenization).
• Employment applications: résumé/CV, licenses, work history, availability, interview scheduling data, and files you upload.
• Marketing preferences: opt-in or opt-out choices for email, SMS, or other outreach where offered.

Message frequency varies. Message and data rates may apply. Consent to marketing texts is not a condition of purchase. You may opt out of marketing texts by following instructions in the message (such as replying STOP where supported) or by contacting us. Transactional texts about an existing booking may still be sent as permitted by law.

5. How We Share Information

We do not sell your personal information for money. We may share information with:

• Service providers who assist with hosting, booking (including Bookly or similar tools), payments, SMS/email delivery, analytics, security, customer communications, accessibility widgets, and IT support, under appropriate contractual safeguards;
• Licensed practitioners and staff involved in your care or visit;
• Insurance, billing, or claims partners when you request or we participate in insurance billing;
• Professional advisors (legal, accounting) under confidentiality obligations;

Aggregated or de-identified information that cannot reasonably identify you may be used for analytics and improvement.

6. Health Information & HIPAA

Health information collected to provide licensed Services may be PHI. We implement administrative, physical, and technical safeguards appropriate to our size and operations. Use and disclosure of PHI for treatment, payment, and health-care operations, and your rights regarding PHI (access, amendment, restrictions, accounting of disclosures, and complaints), are described in any Notice of Privacy Practices provided at the center or upon request. Website inquiry data that is not combined with a clinical record may be treated as ordinary personal information under this Policy, but we still handle it carefully.

7. New York SHIELD Act & Security

We take reasonable measures designed to protect private information as defined under New York’s SHIELD Act and other applicable laws, including access controls, staff awareness, and vendor diligence. No method of transmission or storage is completely secure; please use unique passwords for any online account and notify us promptly of suspected unauthorized access.

8. Retention

We retain information for as long as needed to provide Services, meet legal, tax, licensing, and recordkeeping requirements, resolve disputes, and enforce agreements. Clinical records are retained for periods required by New York law and professional standards. Employment application materials are retained according to our recruiting needs and legal obligations, then securely deleted or archived.

9. Your Choices & Rights

Depending on applicable law, you may have rights to:

• Access or obtain a copy of certain personal information;
• Request correction of inaccurate information;
• Request deletion of certain information, subject to legal retention needs;
• Opt out of marketing emails or texts;
